I collect information such as your name, contact details, health history, and insurance information to provide you with the best care possible. I use this information with Soul Work Counseling to plan your treatment, communicate with you about appointments, and handle billing or insurance matters.

Everything you share with me is private and protected by HIPAA and Minnesota law. I will not share your information without your written consent, except in cases where I am legally required to do so (for example, if there is a risk of harm to yourself or others, or in situations of abuse). If we communicate by email, text, or online forms, please know that these methods may not be completely secure. I do my best to protect your information in all forms.

You have the right to see your records, ask for corrections, and request a copy of your information. You can also revoke consent for sharing your information at any time, except where disclosure is required by law. I take careful steps to protect your information, both digitally and physically. If I ever update this privacy policy, I will let you know about any important changes.
