Requesting A Referral

To schedule an appointment, we will need the following patient information:

  • Full name 
  • Address 
  • Email address
  • Phone number 
  • Date of birth 
  • Insurance information (e.g., HMO, Medical Assistance, Workers’ Compensation, private insurance) 
  • Brief description of the presenting concern or reason for referral 


We also request:

  • Referring provider name and National Provider Identifier (NPI), if applicable 
  • Type of services being requested (therapy, diagnostic evaluation, specialized treatment, etc.) 


Appointment Scheduling

Patients with urgent clinical needs will be prioritized when appropriate. Non-urgent referrals are typically scheduled within several days to several weeks depending on availability. Riverside Counseling primarily provides outpatient mental health services, with telehealth options available for clients across Minnesota.


After the Patient is Seen

Following an intake or ongoing care episode, referring providers (when applicable) may receive:

  • Clinical progress notes and treatment summaries (with appropriate consent) 
  • Updates regarding treatment engagement and care coordination needs 
  • Recommendations for ongoing or collaborative care planning 
  • We encourage referring providers to maintain open communication with our clinical team to support continuity of care.


Care Coordination and Collaboration

Riverside Counseling values collaboration with primary care providers, specialists, schools, and community partners. Our goal is to ensure coordinated, client-centered care that supports long-term stability and recovery. We recognize that clients often maintain ongoing relationships with their primary providers, and we support continuity of care before, during, and after mental health treatment.

Riverside Counseling provides outpatient mental health services focused on evidence-based, trauma-informed care with clinical focus on:

  • Anxiety and mood disorders 
  • Trauma and PTSD treatment
