Serious and Persistent Mental Illness (SPMI)

Support for Living Well With Complex Mental Health Conditions

Living with a serious and persistent mental illness can affect far more than symptoms. It can influence relationships, work, school, housing, motivation, routines, physical health, and your sense of identity.


At Riverside Counseling, we believe you are more than a diagnosis.


Therapy can provide a place to understand what you are experiencing, strengthen coping skills, navigate challenges, and build a life that feels more stable, meaningful, and manageable.


Your diagnosis is part of your story. It does not define your entire story.


What Is SPMI?

Serious and Persistent Mental Illness, commonly referred to as SPMI, describes mental health conditions that can significantly affect a person's functioning and may require ongoing treatment and support. SPMI may include conditions such as:

  • Schizophrenia and other psychotic disorders
  • Schizoaffective disorder
  • Bipolar disorder
  • Severe or recurrent major depressive disorder
  • Other complex and persistent mental health conditions

The experience of SPMI is different for every person. Symptoms, functioning, treatment needs, strengths, and support systems can change over time.


Therapy Beyond Symptom Management

Living with a serious mental health condition is not simply about managing symptoms. Therapy may also focus on:

  • Developing greater emotional stability
  • Recognizing early warning signs
  • Strengthening coping strategies
  • Improving daily routines and functioning
  • Building healthy relationships
  • Managing stress and emotional triggers
  • Increasing self-awareness
  • Working through grief and loss
  • Addressing trauma when clinically appropriate
  • Strengthening communication skills
  • Developing realistic goals
  • Increasing independence and confidence
  • Identifying personal strengths and protective factors
  • Creating a meaningful life alongside ongoing mental health challenges


The goal is not to promise that every symptom will disappear.

The goal is to help you build the skills, insight, support, and stability needed to move forward.


A Person-Centered Approach

People living with SPMI are sometimes defined by their diagnosis, treatment history, or past experiences with the mental health system.

Riverside Counseling takes a different approach.


We want to understand you—including your experiences, strengths, values, relationships, goals, challenges, and hopes for the future.

Your diagnosis provides important clinical information. It does not tell us everything about who you are or what you are capable of accomplishing.

Therapy is collaborative. We work together to identify what is most important to you and determine which therapeutic approaches may be appropriate.

Treatment is individualized based on your symptoms, goals, current level of stability, treatment history, and clinical needs.



When Symptoms Become Overwhelming

There may be times when symptoms become more difficult to manage.

You may notice:

  • Significant changes in sleep
  • Increased isolation
  • Difficulty completing daily responsibilities
  • Increased anxiety or agitation
  • Changes in mood
  • Difficulty concentrating
  • Increased suspiciousness or unusual experiences
  • Changes in functioning
  • Increased substance use
  • Thoughts of self-harm or suicide

Recognizing changes early can provide an opportunity to seek additional support before the situation becomes a crisis.

Therapy can help you identify your personal warning signs and develop strategies for responding to them.


Medication & Coordinated Care

Medication management may be an important part of treatment for some people living with SPMI.

Riverside Counseling does not prescribe psychiatric medication. When appropriate, therapy may occur alongside care from a referal to a psychiatrist, psychiatric nurse practitioner, primary care provider, or other medical professional.

With appropriate authorization and when clinically indicated, coordination with other members of your treatment team may help provide more comprehensive care.

Therapy and medication can serve different purposes and may work together as part of a broader treatment plan.


Building a Life Beyond Your Diagnosis

Recovery does not necessarily mean that you will never experience symptoms again.

For many people, recovery is about learning to recognize changes, respond earlier, develop effective coping strategies, maintain meaningful relationships, and continue pursuing goals even when challenges arise. That may mean:

  • Returning to work or school
  • Strengthening relationships
  • Developing healthier routines
  • Finding meaningful activities
  • Improving self-care
  • Building confidence
  • Reconnecting with interests
  • Establishing greater independence
  • Developing a stronger sense of identity


When Outpatient Therapy Is Appropriate

Riverside Counseling provides scheduled outpatient psychotherapy.

Outpatient therapy may be appropriate when a person is able to participate safely in scheduled treatment and does not require continuous supervision or a higher level of psychiatric care. If symptoms become severe enough that you cannot safely care for yourself, are experiencing an acute psychiatric crisis, or need immediate stabilization, a higher level of care may be necessary. This may include crisis services, emergency psychiatric evaluation, hospitalization, or another level of behavioral health care. Seeking a higher level of care is not a failure. Sometimes additional support is the safest and most appropriate next step.

A diagnosis can help explain part of what you are experiencing.

It does not define your worth.

It does not erase your strengths.

And it does not determine everything that is possible for your future.
